When does a server's Start Command execute?

Does a player have a to request to connect or is the command called once a VM is ready or in standby?

Once the VM is ready, PlayFab will use "Start Command" to create a new server instance. A complete flow is like the following. For more details, see Integrating the PlayFab Game Server SDK.
