If you have a PlayStream Rule configured to trigger an Action on a given player event, it will fire 100% of the time. If the Action is an email, there's an external dependency on your SMTP server - so the short answer would be that the chance of failure is completely down to the reliability of your server. What I can say from our side is that there will be a request sent to it to send the appropriate email every time we get the event.
