The 20 server hours are intended for titles in development to do key testing to determine their usage. All server hours for live titles are billed at the actual usage rate in EC2 plus 10%. You can increase your usage hours on the limits page - go to Settings->Limits in your title, and click on "Game sever hours".
Yes, if you disable the Region in question, that will tell us to shut down all servers in that region. Please bear in mind that AWS does all EC2 billing on a per-hour basis though, so if you run a server for 5 minutes and shut it down, they still count that as an hour.
