I would like to know if the $299 monthly minimum covers only the per MAU fees or if it also applies to other fees. For example, it we pay $19.99 for expanded player inventory but only have 20k MAU ($160), would we be charged $299 or $318.99 in one month?

Well first, there would be no reason to have the expanded inventory upgrade if you were in our Professional tier, as it already has the higher limits for that (and many other things). If there's a limit you want increased beyond what you get in the Pro tier (https://community.playfab.com/questions/13455/limits-for-pro-tier.html), we should discuss specifics.
That said, the $299 monthly minimum - which is specific to the Professional tier - is indeed the monthly minimum for the core service. Add-on Marketplace functionality with costs are distinct, as is server hosting and CDN usage.
Ah, I didn't know those limits were different on the pro tier. Much better actually, definitely should be listed on the pricing page. We are currently using the free tier for development, but knowing the pro tier limits we might switch earlier for the increased limits.
